After a Sightbox member (your patient) has completed their exam and you've charged the card a Care Coordinator called and gave over the phone, we ask that you fax a copy of the itemized receipt and finalized contact lens prescription to 888-540-8723.
If the prescription is not yet finalized, please send the copy of the receipt for services only. Sightbox does not fill prescriptions until they are finalized by the doctor, and we follow the doctor's orders and only fill the brand prescribed.
